Referral patients are almost always pre-eligible for our services. Our team will conduct a full benefits check prior to your session, but you will be notified in the rare event your insurance is not in-network.
Benefits of Working with a Swap Health Dietitian

Almost all Swap Health patients are eligible for unlimited ongoing sessions, at no cost to them, through most major health insurance plans.
Before you get started, our team will check your benefits and let you know how many sessions your insurance will cover, whether or not you will have a copay, etc., In the rare event you are not covered, our team will let you know in advanced and you will not receive any charges.

Your dietitian will help you stay accountable, providing you with motivation and support in a way that works best for you.
If you experience challenges and/or barriers with staying on track with your goals, your dietitian will work with you to figure out ways to gradually overcome your challenges.

Your dietitian will work with you to determine the appropriate duration of your sessions, along with how frequently and when you’d like to meet.
Video or Phone Call Options
You can schedule video or phone call sessions with your dietitian based on your personal preferences and needs.

In addition to having access to general nutrition and lifestyle content, your dietitian will send you specific resources to help you with the goals you are working on. These resources can include everything from nutritious recipes to condition-specific educational materials.

By working with a Swap Health dietitian, you’ll also be eligible for $10 weekly cash back rewards on nutritious grocery purchases to help you with your health goals!

Manage Your Sessions
With the Swap Health app, you can conveniently manage your past and upcoming sessions.
You also will receive notes after each session, along with helpful information about what goals you’re currently working on, what foods you should try, etc.
If you’re not comfortable using an app, or do not have access to a smart phone, our Patient Care Coordinators will help you manage your sessions!

Schedule Your Session with a Swap Health Dietitian
Typically, our Care Team will reach out to you to help you get scheduled if your Health Care provider has referred you to Swap Health. If you would like to reach out to us to get scheduled, you can text or call us at (828) 202-78421 or you can schedule online→!
Once you’ve successfully scheduled your first session, you’ll receive a confirmation email along with text and/or email notifications.
Same-day cancellations or no-shows hurt our ability to help others in our community. Therefore, if you need to cancel or reschedule your session, we ask that you do so 24-hours before your schedule session.

First sessions typically last around 45-minutes to give you and your dietitian enough time to get to know each other and to review what you expect from Swap Health and what your goals are.
After your initial session, if you’d like to request a different dietitian, you can reach out to our Care Team, and we’ll help you find a better fit for you.

After your initial session, you’ll receive your first set of goals to work on between sessions. These goals are typically small, actionable goals that build up to larger milestones. As you continue to progress towards your goals, you and your dietitian will determine new goals or revisit past goals to help you achieve long-term success.

Between sessions, your dietitian will also be sending you important nutrition and lifestyle resources like meal and food recommendations, recipes, etc., to help you stay on track with your goals.
Your dietitian will also send reports to your healthcare provider so you don’t have to be responsible for managing information across your Care Teams!

Many of our patients work with us for years, even after they’ve achieved their initial health goals to help them with maintaining their health.
While newer patients typically see their dietitian on a weekly basis, you and your dietitian can discuss adjusting how frequently you meet according to your needs and progress. We’re here to support you!
While you can opt out of receiving care from Swap Health at anytime, we recommend seeing your dietitian on a regular basis for consistent and sustainable results.

Swap Health works with health insurance plans to make sure that your access to Swap Health is completely covered. That means if you have any insurance coverage through Medicaid, an MCO, or most commercial health insurances, you won’t need to pay for Swap Health. In rare instances, some users have a copay with their insurance but we’ll make sure you know ahead of time if that applies to you.

Yes. If you feel your dietitian is not a good match for you, you can speak with our Care Coordination team, and we’ll assign you a new dietitian better suited to support you.
If you have a particular dietitian you would like to work with, please let us know, and we will try our best to accommodate your requests.

During your first session, you will meet virtually (either by phone or video call) with your assigned dietitian for around 45-minutes. If you have accessibility concerns, please let us know in advance, and we will do our best to accommodate your needs!
You’ll discuss your overall health goals so we can determine how we can best support you and how you can get the most from the Swap Health app.
You can discuss how you’d like to receive support from our dietitians, if at all, and you’ll be given instructions on how to download the Swap Health app so you can start working on your health goals and earning cash back!

For new members, follow-up sessions are generally 30-minutes long and take place on a weekly or bi-weekly basis. Your dietitian will work with you to find a cadence that works for you, so make sure to let your dietitian know if you need something more flexible.
During follow-up sessions, you and your dietitian can discuss your progress and talk about any challenges and/or successes you experienced since your last session. We understand progress is not always linear, so we encourage our members to discuss modifications to their recommendations during follow-ups!

No, members do not have to commit to a certain amount of sessions/appointments, but we encourage members to take advantage of having access to our expert registered dietitians so they can reach their health goals!
Members are, however, required to attend sessions at least once every 8 weeks in order to maintain access to our benefits, like cash back on groceries.
If you schedule an appointment, however, we do ask you to attend your session or cancel within 48 hours of your scheduled appointment to respect dietitians’ times and to allow our other members to schedule appointments.

In order to receive cash back on groceries, you’ll be asked to upload your receipts so we can validate which items are eligible for cash-back. Once you’ve uploaded your receipt(s), our team will send you up to $10.00 cash-back (each week) on eligible grocery items via an E-gift card for your favorite grocery store.

Your health coach and/or registered dietitian will work with you to identify foods that are considered healthy for your personal health needs and goals.
If you purchase groceries within the list of recommended items that are provided for you (and provide us with a copy of your receipts), we’ll send you cash-back on those items!

Don’t worry, our platform was designed to be extremely easy to use and takes only a few minutes to get up and running. If you’d prefer to not use an app on your phone, you can also access SwapHealth directly from a laptop or desktop computer.
